To ensure the smoothest experience possible, we kindly ask that you provide:
Your dental insurance information (if available)
Any previous X-rays or records from your last dental provider (if seen within the last 5 years)
A list of any concerns or symptoms you may be experiencing
Our team will verify your insurance coverage and go over all costs prior to performing any procedures, so there are no surprises.
